Employment Services Organization
“Employment services organization” means an organization that provides community-based employment services to individuals with disabilities that is an approved Commission on Accreditation of Rehabilitation Facilities (CARF) accredited vendor of the Department for Aging and Rehabilitative Services.
To apply for ESO Certification, you will need:
- Copy of Certification letter or Certificate from Virginia Department for Aging and Rehabilitative Services (DARS)
- Copy of Certification letter or Certificate from CARF
- Copy of IRS Form 990 of most current year
- Copy of Organization By-laws
